In order are the 11 dynasties:

 

1. Shang Dynasty: c. 1600–1046 BC
2. Zhou Dynasty: c. 1046–221 BC
3. Qin Dynasty: 221–206 BC
4. Han Dynasty: 206 BC – 220 AD

• The Legend of Chu Qiao - early Han Dynasty
5. Jin Dynasty: 265–420 AD
6. Sui Dynasty: 581–618 AD
7. Tang Dynasty: 618–907 AD
8. Song Dynasty: 960-1279 AD 
9. Yuan Dynasty: 1271–1368 AD
10. Ming Dynasty: 1368–1644 AD
11. Qing Dynasty: 1644–1911 AD

(*) My understanding of the connections between the dynasties and the novels is based solely on the events described in the stories and my limited knowledge of Chinese history. Please do not take this interpretation for granted; even if some authors provided hints, they may or may not have confirmed or stated otherwise.
